Getting married or having a baby? Access the LyondellBasell Benefits Portal at www.lyb.com/benefits, or call 888-596-8008 and select option 00. Changes must be reported within 31 days of the event.
Need a medical or dental ID card? Access the LyondellBasell Benefits Portal at www.lyb.com/benefits. Click “Main Menu,” select your vendor, and follow the prompts.
If you need to speak with someone, call the LyondellBasell Benefits Service Center at 888-596-8008 and select one of the following options:
- Option 00 for Health and Welfare
- Option 12 for LTD, STD and FML
- Option 14 for Medical
- Option 15 for Dental
- Option 16 for Vision
- Option 17 for Voluntary Benefits
- Option 18 for Employee Assistance Program
- Option 19 for Prescription Drug Benefits
- Option 20 for COBRA
- Option 21 for Health Flexible Spending, Dependent Care, and Health Savings Accounts
- Option 22 for Life Insurance/AD&D
